Above link is the indemnity form for 1 Star Kayaking Course.
For those whose names appear in the namelist, please make sure you print out a piece for yourself and have it duly filled

This is an important and Compulsory form to be brought down on the day of the course.
However, if you forget/did not bring the form on the day, you may not be allowed to take the course due to uncertain individual safety issues.

So please, do not say i did not remind you guys & girls.

A Brief Overview:

Venue : Singapore Dragon Boat Association ( Kallang )
Time : 1st grp: 0800-1300/2nd grp : 1330-1900

Checklist to bring on the day itself:
-one set of clothing (eg. T-shirt, shorts) for kayaking,
- footwear such as sandals or sport shoes (avoid slippers),
- straps for those wearing spectacle as required to do capsize drill,
- water bottle,
- sun screen lotion, basic toiletries, towel, and a plastic bag for wet clothing
- completed indemnity form
